Academic Requirements
All Teaneck High School student-athletes must meet the academic requirement as set forth by the Teaneck Board of Education and the New Jersey State Interscholastic Athletic Association (NJSIAA). This policy stipulates that student-athletes must meet the following criteria:
Credit Requirement:
- All Sophomore, Junior and Senior students must have earned at least 30 credits the previous year to be eligible to participate in fall and winter sports. To be eligible to participate in a Spring sport, students must earn at least 15 credits in the first two marking periods of the current year.
- Freshmen will not have to meet a credit requirement for Fall and Winter sports participation, however, they will have to earn at least 15 credits in the first two marking periods of the current year to be eligible to participate in a spring sport.
Any student who fails to meet either the credit requirement will be ineligible to participate in the Teaneck High School athletic program even if they have been medically cleared.
Medical Requirement
The New Jersey Department of Education, in accordance with the Scholastic Student Athlete Safety Act, has implemented a revised Pre-participation Physical Evaluation Form (PPE) which is to be utilized by all schools beginning May 2014. THIS IS THE ONLY ACCEPTABLE FORM FOR ATHLETIC MEDICAL CLEARANCE! The PPE form includes a History section, to be completed and signed by the parent/guardian; a supplemental history form for students with special needs (if applicable); and a physical examination section and clearance form to be completed by the examining home physician, schools physician, advanced practice nurse (APN) or physician assistant (PA). Please note that Teaneck High School requires your doctor to sign and stamp all physical forms.
In addition to the PPE forms, student-athletes and their parent/guardian are required to review and acknowledge the following documents:

Students will register on our new Medical Clearance platform FormReLeaf at
FormReLeaf.com.
STUDENTS MUST BE BOTH MEDICALLY AND ACADEMICALLY CLEARED IN ORDER TO BE ELIGIBLE TO PARTICIPATE IN ANY ATHLETIC PROGRAM AT TEANECK HIGH SCHOOL.
