Teaneck High School along with Benjamin Franklin and Thomas Jefferson Middle Schools will be using FormReleaf in order to register for athletic participation.  Please make sure all information is accurate and is completely filled out by a parent/guardian. Student/athletes must register for EACH sport they plan on playing during the 2025-2026 school year

Make sure you have a current physical (within 1 year of the start of your season) or you can upload a new physical onto your FormReleaf registration. Our school doctor will also be here for physicals every Thursday or Friday morning. You need to make an appointment in the nurse's office for a physical. You must be registered on FormReLeaf to get a physical with our doctor.

This is the physical form that needs to be filled out by the doctor's office if you get your own physical. Please make sure that you have the correct form especially if your doctor is located outside of NJ.

Please don't wait until your season is about to start. Get cleared now so you are ready to go on the first day of tryouts. Registration is now open for all sports in the fall, winter and spring seasons.
